Forms !  It seems that the entire world is centered around the "red tape" of forms.  But it is necessary.   Thank you so much.  The information below is for any riders...Campers, Girl Scouts, Riders, and boarders of the farm.

Insurance Release Form 2012

Please download this form if you are riding on the Farm.  Girl Scouts, Campers, and riding students must have one on file before they will be permitted to ride.   If you are a mother with a child (or children) riding in a program, you may put the names of the same family on the same form.   This will make the form usable for everyone that are listed on the back of the form.  Remember that this form will download in two (2) pages, so be sure to fill out your address, names of rider (s), and email address.  Your phone numbers are very important.  Please bring this filled out form with you to your riding program the first time you come.  I will keep it on file for you during the current year that you have dated on the form.   Both biological parents must sign this form.  If there is a divorce or separation in the family, you still need to get the signature of the biological parents..(both)..   Fax a copy, or have your spouse download this form from my web site (www.cdifarm.com) and then fax or mail to you to bring with you to the riding program.   This is our legal policy, and we can't deviate from it.  I want to thank you in advance for the work you put into these forms for your riders/campers/ and girl scouts.
