This medical form is for riders who will be attending a week-long, or longer, camp at CDI Farm.   Please fill out all of the information and phone numbers.   Along with this form, it would be very helpful if you photo copy your Insurance card in case of an emergency.  The hospital has asked that I attach a copy of this card to the forms so that immediate attention can be given to the care of any injured rider.   Please know that parents are always the first to be contacted, but in case I can't reach you, your rider will be taken care of by the Ashtabula Medical Center in Ashtabula, Ohio.
